1. Identify the reel number(s) on all reels to be returned.

2. Call 800-631-7188 and report the reel number(s).

Print and Fax the form

1. Click to view/print the Steel Reel Report Form (NAS036).

2. Record the reel number(s) on the form.

3. Fax the form to Okonite's Traffic Department at 201-236-3071
